- Activity Level: High, Moderate, or Low
- Medical Needs: Low to Moderate
- Behavioral Needs: Low, Moderate, or High
- Mental Health Needs: Low to Moderate
- Supervision Needs: Low, Moderate, or High

- Primary diagnosis of Intellectual Disability (ID) and/or Developmental Disability (DD)
- Active Support Coordinator
